Elemental Assessment, Your Intro to Functional Medicine
The Elemental Assessment provides an introduction to the functional medicine experience, including intake, lab review, and a comprehensive plan tailored just for you. Expect the following:
- Functional Health Evaluation
- Lab review of Advanced Biomarker Testing: in-depth blood panel measuring inflammation, nutrient status, thyroid function, metabolic health, hormone balance, and more (labs are billed through insurance or can be purchased at an affordable cash pay rate)
- Extended Virtual Appointment with Dr. Ashley Shrader, IFMCP, DNP
- Personalized comprehensive health plan
- Discussion on the structure of future visits, if necessary
- Payment options including HSA/FSA; super-bill can be provided for possible insurance reimbursement
$375 | 90 minutes
Follow-Up Lab Review (Nonmember)
Expect a thorough follow-up on how your symptoms are changing and evolving. During these visits, we can address and review your diagnostic testing. We can review up to two lab orders per visit or spend extra time evaluating your symptoms and developing your plan. Note that this visit is for non-members.

Follow-Up Visit (Empower Member Only)
Follow-Up Visit for Empower Members ONLY. This visit is available exclusively to members who have signed up for the $75 monthly membership. To set up this membership option, please email ashley@risefxmed.com.

About Ashley
Ashley Shrader, DNP, is a Family Nurse Practitioner and IFM Certified Practitioner. Prior to functional medicine, Ashley worked in various conventional healthcare settings, which revealed the algorithmic and impersonal nature of the traditional healthcare model that leaves many patients overlooked and disempowered. Dissatisfied and in the midst of her own health crisis, she began researching other approaches and discovered functional medicine.
She pursued certification with the Institute for Functional Medicine and eventually established her own practice, Rise Functional Medicine. Ashley has a passion for gut and hormone health, especially as they relate to brain and mental health. As a Yoga Teacher, Ashley believes that mental, emotional, and spiritual health are the foundation for the path to healing.
Her career in healthcare reflects her own personal health and wellness journey, which includes getting diagnosed with autoimmune disease and learning she has a genetic predisposition to developing ALS (Lou Gehrig's disease).
